To format this manual afresh, you'll need to get up-to-date copies of
all the style options used and have recent copies of makeindex and
bibtex. Some figures are included as PostScript files using `psfig'.
Originally for the printed version it was set mainly in Palatino and
LucidaNewMath, which you likely don't have. The PostScript for this
isn't distributed as it contains proprietary fonts. The distributed
PostScript was generated by re-setting it in Times and Computer Modern
to less effect and assuming a 300 dpi write-white printer with A4 paper.
I'm, not sure whether the PostScript will adjust to the absence of an
A4 tray or not. If not, sending it to the printer preceded by the
PostScript fragment
  0 36 translate
should shift it more suitably for a US letter page.

[The definitive source of the LaTeX add-ons etc. is the Comprehensive
TeX Archive network: anonymous ftp to ftp.shsu.edu (US) or
ftp.tex.ac.uk (Europe) plus ftp.dante.de (Germany).]
